Fee Waiver Registration Form

Based upon the fee waiver calculator, it appears that you qualify for the waiver. Please complete the following form, upload a PDF, JPG, GIF, DOC or TIF of the supporting documentation, and click submit.

Once we confirm your qualifications, we will register you for the requested online course and send to you your user ID and password, along with instructions for completing the program.

INCOME VERIFICATION REQUIRED*: Please note that in order for us to process your fee waiver request, you are required to submit income verification documentation (mailed, scanned, email or shared from your phone). Such documentation may include any one of the following:

  • Alimony letter or receipt
  • Child support letter or receipt
  • Letter from your attorney certifying your income
  • Paystub(s) for the last 30 days
  • Schedule I from your bankruptcy petition paperwork
  • Self-employment income (tax form)
  • Social Security, SSI or SSD award letter
  • Unemployment award letter
  • Veterans or pension benefit letters

Files may be in PDF, JPG, GIF, PNG, TIFF. DOC, or DOCX format. Your request will NOT be considered complete, nor will it be processed, without income verification documentation. If you have no income, you may submit a note or letter that includes your name, address and phone number, and a statement that you have no income.
